Each family's commitment is cash or a new item donation worth at least $300 which is due no later than January 30. 
For families with children only in Preschool and/or Pre-K, the commitment is cash or a new item donation worth at least $150.  

Preschool/Pre-K families: select quantity 1 to pay $150 commitment.
K-8 families: select quantity 2 to pay $300 commitment.

I authorize Our Lady of Fatima - Seattle to use the credit card entered on the submission page at event Fatima Flashback Formal 2026. Once the transaction has completed successfully, a confirmation receipt will be sent to the email address entered.
